*Job Description:*
In this role, you will provide direct patient care, including comprehensive assessments, treatment planning, medication management, and follow-up care. The ideal candidate has experience in addiction medicine and is dedicated to supporting individuals on their path to recovery.
*Status: *Independent Contractor
*Location*: 3571 Muskoka District Road 169, Bala, Ontario
*Hours*: ~ 20 hours per week
*Responsibilities:*
· Provide direct, in person patient care, including conducting comprehensive assessments, establishing diagnoses, and formulating treatment plans.
· Prescribe and manage medications, monitor efficacy, and adjust treatment as necessary.
· Collaborate with Care Teams to develop, implement, and assess patient-centered treatment plans.
· Educate patients and their families about addiction, recovery, and related health issues.
· Monitor and evaluate patient progress, adjusting treatment plans as necessary.
· Stay abreast of best evidence-based practices for addiction medicine.
· Collaborate and support other departments to ensure high quality client care i.e. medical director/psychiatry
· Collaborate with the admissions team to oversee admission, nursing, and patient care processes.
· Mentor/provide guidance to medical team around client care and best practices
· Advocate for the rights and needs of patients.
· Ensure nurses are in compliance with Provincial and College regulations, with health and safety standards, as well as maintaining the highest standards in nursing care.
*Requirements:*
· 3 + years nursing experience, with mental health and addiction expertise preferred.
· Current registration in the Extended Class (Nurse Practitioner) with the College of Nurses of Ontario is required.
· Active membership in related professional associations required, e.g. NPAO, RNAO.
· Mental Health Nursing certificate preferred.
· Master of Nursing preferred.
· Innovative, creative and professional role model.
· Possess excellent assessment and clinical skills.
· Ability to prioritize/organize multiple demands.
· Ability to work autonomously.
· Working knowledge of Mental Health Act and community mental health resources.
· Highly skilled in assessment and treatment of psychiatric disorders, including PTSD, mood and anxiety disorders, psychotic disorders, delirium, depression, suicide prevention, and the management of aggressive behavior.
· Self-driven motivated learner in basic primary care, in addiction care, mental health care, and the physical causes of psychiatric symptoms
_*The Nurse Practitioner will be retained on a fee for service contract.*_
