*Position Summary*
The Registered Social Worker is a member of the Leadership Team, reports to the Executive Director and works with residents and their families/caregivers at Chapman House to ensure their psycho-social-emotional well-being is supported. This Support Services Lead supervises Spiritual Care, the Complementary Therapy Program and Grief and Bereavement Services volunteers.
*Description*
The role of the Grey Bruce Hospice Social Worker is to advocate for each resident?s end-of-life wishes and help individuals address the emotional aspects of death and dying. The GBH social worker supports the emotional, psychological, and social wellness of end-of-life care for our residents and their families/caregivers. This staffer will also make connections to other support services for both the family and resident. The social worker helps residents, and their families navigate planning for end-of-life care and will assist in managing the stresses of illnesses, including emotional, familial, and financial; as well as overcome crisis situations. The social worker will actively engage with potential residents as of the time they qualify for admission (3months life expectancy) in preparation for transition into Chapman House, as well as when in residence. As the primary staffer responsible for grief and bereavement programs, the social worker to design, implement and oversee volunteers for the growing grief and bereavement programs at GBH.
*Key Duties & Responsibilities*
* Receive client referrals and complete biopsychosocial assessment to determine the best hospice care solutions for the resident.
* Provide information and supportive counselling to persons on the GBH waitlist in their last months of life, as well as to their family members/caregivers, through office visits, home visits, phone consultations, email, and/or follow-up.
* Offer families/caregivers resources to support services in the community.
* Provide emotional support. Guidance and help resolve psychological crisis.
* Intervene and assist in conflict resolution discussion between families/caregivers.
* Follows care plans for Chapman House residents, covering psycho-social-emotional domains of care.
* Attends weekly rounds with inter-disciplinary team - Clinical Coordinator, Medical Director/Physicians, RNs, RPNs, PSWs and Spiritual Care Coordinator.
* Conduct regular check-ins with Chapman House residents and families to determine status, and to support Clinical Team with non-medical care.
* In conjunction with the Clinical Coordinator, assist residents/families with paperwork and documentation ? POA, admission paperwork, facilitates contacts with financial advisors/lawyers, etc., both prior to admission, or when in residence.
* Help recruit Complementary Therapy practitioners, and promote to residents and family/caregivers.
* Conduct InfoAnywhere check-in?s with residents/families.
* Coordinate discharge and disposition planning including coordinating MAiD provision and LTC transfers.
* Plan and facilitate groups and programs in grief, anticipatory grief, and bereavement within GBH for residents and their families/caregivers, and when space allows, the general public.
* Provide supportive counselling, lead family meetings and difficult conversations with challenging residents and families.
* Support, train, and monitor the bereavement volunteers.
* Provide supervision to social service workers and/or social work students when applicable.
* Maintain client files and statistical data in accordance with organization policy requirements.
* Refer to and follow appropriate legislation pertaining to the Ontario College of Social Workers.
* Maintain current knowledge of palliative care and relevant community resources, and participate in continuing education.
*Educational Requirements & Experience*
* Regulated Health Professional in good standing and registered with the Ontario College of Social Workers, or equivalent.
