Our medical clinic is currently seeking a dedicated NP to join our team as a member of the Nurse Practitioner Primary Care Program. As a Nurse Practitioner at our clinic, you will play a crucial role in providing high-quality patient care and promoting wellness within our community.

· We will assist our Nurse Practitioner in signing up for the Nurse Practitioner Primary Care Program and will assist the NP in fulfilling the ongoing program requirements per the program guide.

*\*\*Key Responsibilities:\*\**
- Conduct comprehensive patient assessments, including medical history review, physical examinations, and diagnostic testing.
- Develop and implement individualized care plans in collaboration with patients, families, and other healthcare professionals.
- Diagnose and treat common acute illnesses, injuries, and chronic conditions, including but not limited to hypertension, diabetes, and respiratory infections.
- Prescribe medications and perform minor procedures within the scope of practice.
- Educate patients on preventive care, health maintenance, and treatment options to empower them to make informed decisions about their health.
- Maintain accurate and up-to-date medical records, including patient charts, progress notes, and treatment plans.
- Collaborate with physicians, nurses, and other interdisciplinary team members to ensure continuity of care and optimal patient outcomes.
- Participate in quality improvement initiatives and adhere to regulatory standards and best practices in healthcare delivery.
- Maintain a minimum patient panel of 900 patients. Prior to having a patient panel of 900 patients, NP must provide walk-in availability
- Utilize CII/CPAR to manage patient panels
- Share patient panel with HQCA for quality improvement (Panel reports)
- Provide Alberta Health with yearly annual audited financial reports that are in accordance with Canadian Accounting Standards for not-for-profit organizations.
- Report quarterly on key performance indicators that measure the progress and quality of primary care delivery in their settings ? performance measures and evaluative processes will be developed by Alberta Health.
- Hold adequate personal liability protection with CNPS

- Current licensure as a Nurse Practitioner valid in Alberta.
- Strong clinical assessment and diagnostic skills
