NLHealthServices.ca Job Summary The primary purpose of this position is to apply advanced clinical knowledge and skills in the treatment of referred clients for Case Management Services. The Social Worker works in collaboration with the community mental health and addictions team and other case managers within the Program. The Social Worker must be skilled in psychosocial assessment, counselling with individuals and families, crisis intervention and discharge planning. The successful applicant will work as part of an interdisciplinary team. The Social Worker will also be expected to provide field instruction for Social Work students.This position works with considerable independence and carries an active caseload and other duties as assigned in the Case Management Program. This position practices using the recovery model of case management based on the Flexible Assertive Community Treatment model and receives support on the model from the clinical supervisor in case management. This position may also be required to work at the DoorWays Walk-In Counselling Clinic.This position reports to the Program Manager for the Community Mental Health and Addictions Team and maintains accountability to the Professional Practice Consultant for Social Work for integrity and professional practice issues. Job Demands By applying for this job, you acknowledge an understanding that regular requirements and demands of this job include (but are not limited to):Occasionally, there is lifting or physical strength required to lift or carry objects up to 10 lbs.Regularly sits to consult with clients, conduct therapeutic sessions, and to work on the computer.Occasionally works in awkward or cramped body positions; pushes, pulls, and sets up furniture for group work, and drives to conduct home visits with clients.Regularly uses fine finger/precision work on a computer. Job Qualifications Graduation with an accredited Bachelor of Social Work Degree is required.Current and continuing registration with the Newfoundland and Labrador Association of Social Workers (NLASW) is required.Minimum of two years of experience within the last four working in a mental health and addictions program is required.Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.Knowledge of community resources, systems and programs.Knowledge of mental health and addictions issues, treatments and available services.Knowledge of specific mental health approaches including supportive counseling and crisis intervention.Knowledge of case management.Demonstrated ability to direct, supervise and mentor students.Demonstrated clinical skills in assessment and interventions with individuals, families and groups.Demonstrated analytical, critical thinking, p
