Nurse Practitioner (NP) Led clinics are an innovative model for delivery of comprehensive primary health care in Ontario and Canada. This model is designed to improve access to care for the thousands of individuals and families who do not currently have a primary health-care provider. One of the unique aspects of the model is the incorporation of nursing leadership within an interprofessional team. NP clinics have interdisciplinary teams consisting of Registered Nurses, Registered Practical Nurses, Social Workers, Dieticians, and Pharmacist all working together with our patients to provide a holistic approach to healthcare.
*Position Overview:*
The Social Worker is a part of the interprofessional primary health care team and provides professional psychosocial services. The goal of social work practice is to restore, maintain and enhance emotional and social functioning by mobilizing strengths, supporting coping capacities, modifying ineffective patterns of relating and behaving, linking people to necessary resources, alleviating environmental stressors and providing psychosocial education related to wellness and subjective well being
*ROLES AND RESPONSIBILITIES*
· By referral, the Social Worker will apply knowledge to assess, diagnosis, treat and evaluate individual, interpersonal and societal problems. Recommendations are made to the patient and nurse practitioner
· Advocacy to establish and access needed resources
· Health promotion
· Psycho-education related to the prevention of mental health problems
· Crisis intervention with patient/families
· Documents care provided in keeping with policies and procedures of the SDNPC
· Sees clients at the Sudbury District Nurse Practitioner Clinics with scheduled and urgent appointments and in the community through outreach and home visits during regular and extended hours.
_Team and Centre Responsibilities_
· Promotes and participates in an interprofessional provider model. Collaborates with both providers and clients to ensure that appropriate management, referral and responses to inquiries are provided
· Participates in the development and evaluation of policies, protocols, and procedures to improve client care and/or promote co-operative and efficient staff functioning.
· Contributes to the development of program plans and develops, plans, implements, coordinates, and evaluates assigned projects in consultation with other team members.
· Participates in the education of students
· Ensures that required statistics are recorded and participates in their analysis
· Contributes where necessary to evaluation processes
· Attends CME and rounds
· Performs other duties as assigned
*SPECIFICATIONS*
· Bachelor degree in Social Work
· Current registration or eligibility for membership with the Ontario College of Social Workers and Social Service Workers
· Two to three years experience in clinical counseling and community social work
· Demonstrated commitment to community-based health care, and knowledge of community-based resources
· Demonstrated computer skills
· Demonstrate thorough knowledge of the theories and practices of professional social work
· CPIC required
