Responsibilities:
* Provides client?centered comprehensive primary and episodic health care to clients, as outlined and appropriate within the Extended Class scope of practice
* Facilitates discussion with clients to determine existing and potential health concerns with a focus on related lifestyle, psychological and environmental risk factors which may influence the client?s health status
* Provides advanced nursing care and treatment; monitors ongoing therapy of patients; implements strategies to promote independent and healthy lifestyles.
* Works within established care pathways, medical directives, guidelines and protocols.
* Orders appropriate investigations and interprets results of screening and diagnostic laboratory tests; determines need for diagnostic imaging, and orders and interprets reports
* Works independently and collaboratively with members of the interdisciplinary team, consulting with other health care professionals when client?s condition is beyond the RN(EC) scope of practice
* Completes follow-up required by client and/or refers when appropriate. *Assists with lab tracking and follow up*
* Maintains complete and accurate records of clients? visits
* Maintains data required by the program, funders, and AHT management
* Participates in regular quality of care chart audits and chart reviews as demonstrated in EMR documentation
* Performs other related duties as required
Qualifications & Experience:
* The Nurse Practitioner must be registered with the College of Nurses of Ontario and be registered as a Registered Nurse in the Extended Class [RN(EC)] and possess a current BCLS. Preference will be given to candidates who possess, or are currently pursuing a Master?s in Nursing or related graduate degree
* CNA certification as a Psychiatric Mental Health Nurse is an asset
* At lease 3 years or demonstrated experience / expertise working as a Nurse Practitioner in a community-based setting
* Extensive knowledge of clinical best practices for the care and treatment of clients with chronic and complex mental health issues/addiction issues.
* The ability to work independently and as a member of an interdisciplinary team, and a demonstrated ability to effectively and professionally carry out the responsibilities of the role is required, including demonstrated ability to establish and maintain effective therapeutic relationships with complex clients
* Experience and knowledge of issues affecting Indigenous people in an urban setting.
* An understanding and working knowledge of health and other issues facing the Indigenous community such as the effects of the post-colonial and residential school syndromes
* Excellent communication and interpersonal skills; demonstrated knowledge of mental health and addictions issues; and extensive knowledge of the clinical standards related to the scope of practice of Nurse Practitioners
* Experience using an Electronic Medical Records (PSS EMR) system for client charting documentation
