Fulfill the mandate of the Walpole Island First Nation (WIFN) Community Health Program (CHP) by participating as a full-functioning member of the Community Health Care Team. Work in accordance with the WIFN philosophy, policies, and regulations; the College of Nurses of Ontario (CNO) Standards of Practice for Nurses in Ontario; and Scope of Practice for a Community Health Nurse (CHN).
*Requirements:*
* Minimum of RN registration with a BScN or BSN from a recognized university.
* Minimum of 5 years of nursing work experience in a relevant healthcare team setting.
* Minimum of 5 years of experience working in a First Nation Community or with First Nation clients.
* Current College of Nurses of Ontario Registration.
* Current RNAO Membership with LAP Insurance.
* Experience in conducting individual and family health assessments.
* Experience in group facilitation an asset.
* Knowledge, skills, and experience with computer programs such as Microsoft Office, Word, Excel, etc.
* Current Canadian Red Cross CPR/First Aid Instructor Certificate or willingness to obtain upon hire.
* Crisis and Suicide Intervention Training Certificate an asset.
* Knowledge and experience with the components of an integrated quality management framework, such as risk management, client safety and quality improvement.
* Valid Ontario Driver's License and a reliable vehicle.
* Subject to criminal reference and vulnerable sector check.

*Responsibilities:*
* Fulfill the mandate of the Community Health Program (CHP), ensuring that Mandatory Health Programs are delivered in accordance with current, relevant health care policies, protocols and regulations.
* Maintain a current CNO Registration and RNAO Membership with LAP Insurance.
* Maintain professional work relations with other WIFN programs and participate in relevant service-integrated initiatives/programs.
* Assist in coordinating CHP services by participating in setting program priorities; reviewing, implementing, and evaluating program work plans/activities; and ensuring safe, effective, and efficient service delivery to community members in accordance with current, relevant health care standards.
* Maintain a current, accurate knowledge of health promotion, disease prevention, and community health nursing issues/practices by participating in relevant professional development activities and continuing educational resources as required.
* Conduct home visits within the community for purposes such as hospital discharge follow-up, individual/family health assessments, identifying client service needs, developing client care plans, providing health education, and initiating appropriate referrals.
* Maintain a resource network system with relevant service providers and agencies.
* Provide age-appropriate, preventative immunization services to community members based on current FNIHB protocol, policies and medical directives and Canadian Immunization Guidelines.
* Conduct Public Health (PH) follow-ups on STI, Communicable Disease, TB contact tracing and DOT, and other PH referrals as required.
* Provide CPR and First Aid Certification/Re-certification instruction to community members based on current Canadian Red Cross standards.
* Provide appropriate professional resource support services to other WIFN programs.
* Occasionally perform acting supervisory duties in the absence of the Nurse Manager.
