Yaletown House is a non profit care facility for seniors with complex care needs and is located in Vancouver, BC. We provide quality health care and services to 127 senior residents in a safe, supportive and home like environment.
Reporting to the Manager Social Work & Therapeutic Services (Manager), and in accordance with the Mission, Vision and Values, policies and procedures and strategic directions of Yaletown House, the Music Therapist is responsible for the provision of music therapy programs and services within the context of a person-centered model of care.
The Music Therapist is responsible for developing, implementing, and evaluating the resident music therapy programs and for providing music therapy services to promote, maintain, and restore resident?s mental, physical, emotional and spiritual health. The Music Therapist works independently and as a member of an interdisciplinary team. According to residents? interests and cognitive and physical abilities, plans appropriate activities and encourages participation using various musical instruments and techniques to enable residents to achieve maximum potential for independence, life skills development and improved self-esteem. Uses music to enable residents to participate with their own self-expression and creative style. Performs assessments on the resident?s needs, develops and implements care plans and intervention strategies, evaluates the effectiveness of the interventions, and communicates and consults with the resident/family and clinical team members as required.
The Music Therapist is also responsible for the planning, development, implementation and evaluation of the continuing music therapy programs to support and ensure quality standards are met, maintained and that the residents? holistic music needs are met.
The Music Therapist is an integral member of the Yaletown House interdisciplinary team and requires teamwork, communication and cooperation. It is the expectation that the individual in this position contributes to a positive, safe working environment, takes instruction well and cooperates with the Clinical Leadership Team and co-workers. This person must manage their time well and participate in meetings and education as required.
*DUTIES & RESPONSIBILITIES:*
* Responsible for providing professional music therapy services and programs to support the residents in providing quality music therapy services and programs within the context of a person centered model of care.
* Develops, implements and evaluates music therapy programs for residents to ensure a high standard of music therapy services and programs are provided that are in compliance with legislative and regulatory acts governing long-term care and the care home standards and expectations.
* Conducts assessments utilizing established standards and guidelines of the residents music therapy needs within the first 2-4 weeks of admission and thereafter quarterly, annually and whenever a referral is received.
* Assessments include assessing resident?s physical abilities, needs, and goals utilizing an evidence-based approach by methods such as reviewing records, gathering information, interviewing the resident, selecting and applying evidence-based assessment methods, administering standardized tests and measures, identifying the resident?s desired program participation and quality of life, and monitoring the resident?s health for significant change to ensure active participation in programs.
* Based upon assessment and reassessments, develops and implements resident music therapy goals and care plans, utilizing appropriate standards and guidelines as set by the Music Therapy Association of BC (MTABC).
