*Social Worker* *(Full-Time)* At Community Care Durham, coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people who choose to live at home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently. Reporting to the Vice President of Client Services, the Social Worker will provide social worker services across the Durham Region supporting clients of the Adult Day Program (ADP), Supportive Housing (SH), Assisted Living Services (ALS), and other Community Support Services (CSS). This individual provides support and resources to individuals in the community to promote well-being and address social and emotional support. This role works collaboratively with Community Care Durham (CCD) departments, community organizations, agencies, and stakeholders to enhance the quality of life for CCD clients. Responsibilities include but are not limited to: * Completes thorough client assessments while providing support and counselling; * Participates in the personal development of clients; * Works in a manner to ensure safety to oneself, clients, colleagues and the work; * Advocates for clients? needs and provides resources accordingly; * Maintains an awareness of community resources, recommends strategies to bridge the identified gaps and assists patients in establishing linkages with community resources; * Provides education to clients and caregivers along with links to community agencies and or resources when appropriate; * Completes and maintains documentation related to assessment findings and care plans. *Qualifications required:* * Bachelor of Social Work or relevant education * Registration with the Ontario College of Social Workers and Social Service Workers in good standing is required * A Master?s in Social Work is an asset * Demonstrate knowledge of mental health, and relevant policies and regulations, such as the Mental Health Act, and Personal * Three (3) years of experience relative to role is an asset * Knowledge of the legislation, regulations, and policies applicable to the practice * Experience working in a community setting is preferred * Proficiency with Microsoft (Word, Outlook, etc.,) * Be able to manage time and workload effectively * Have excellent interpersonal skills and ability to build professional relationships with coworkers and clients * Satisfactory Police Vulnerable Sector Check * Sensitivity to adults with needs related to aging, physical and/or mental health * Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies *Compensation: *$65,000 annually; 3 weeks? vacation per year of work (pro-rated to hours worked); 9 paid (including stat) holidays, 3 paid float days; health, insurance and pension benefits when eligible. \* Pay will be commensurate based on education and experience *Please apply in writing referencing posting #2024-104 outlining qualifications and experience by 4pm on Tuesday November 5th, 2024 to* *Human Resources, Community Care Durham email: careers@communitycaredurham.on.ca * Job Type: Full-time Pay: $65,000.00 per year Benefits: * Company events * Dental care * Employee assistance program * Extended health care * Vision care Schedule: * 8 hour shift Work Location: In person Application deadline: 2024-11-05- LocationWhitby, ON
