ConnectWell Community Health is seeking a Registered Nurse for 28 hours per week (0.8 FTE) to join our inter-professional team. This is an ongoing position (0.8 FTE) which includes a comprehensive employee benefit package and HOOPP. This is a strong possibility this could move to 35 hrs/week, 1.0 FTE. *Position Summary: *We seek an experienced Registered Nurse (RN) who is passionate about working with young families and has a strong interest in pediatric health. As a Renfrew County Child Wellness Hub RN, you will promote the well-being of individuals, families, and the community. Working within a dynamic, interdisciplinary team, you will help develop and implement a new program to improve access to primary care, address social determinants, and promote health for young children and families. The ideal candidate will be committed to making a meaningful impact, responding to social barriers to health, and delivering compassionate, high-quality care. You will thrive in a collaborative setting and contribute to innovative programs integrating primary care with social support services. Responsibilities include providing direct primary care, offering clinical expertise to the team, co-developing health promotion initiatives, collaborating with community partners for timely immunizations and wellness checks, managing episodic care needs, and maintaining clinical resources. Qualifications: * BScN preferred or equivalent combination of education and experience * Current registration with the College of Nurses of Ontario * 3 to 5 years of experience preferably working in a multi-disciplinary team * Excellent health assessment and clinical skills. * Demonstrated ability to work effectively as a member of an inter-disciplinary team. * Experience in the development and delivery of health education and health promotion programs. * Excellent communication skills, both oral and written * Experience working with children ages 0 to 5, an asset * Excellent organizational and interpersonal skills * Experience and understanding of rural communities and rural health needs Reporting Relationship: The successful candidate will report to the Nursing & Admin.
