*POSITION: *Nurse Practitioner *SALARY: * $64.61/ hr *STATUS: *Full Time *HOURS OF WORK: *37.5 hours per week *LOCATION: *Brockville, Ontario *HOURS OF OPERATION: *Weekdays and some evenings - Flexible Work Week *REPORTS TO: *Clinical Program Manager *UNION ASSOCIATION: *OPSEU *Summary of Job:* The Nurse Practitioner will work collaboratively with the Allied Health Professionals and provide comprehensive primary care services including health promotion, disease, and injury prevention, treatment, cure, rehabilitation and support for patients. *Qualifications and Responsibilities:* * Current registration and in good standing with the College of Nurses of Ontario * Strong computer skills * Ability to work well independently * Good organizational skills, accuracy and attention to detail * Strong interpersonal and communication skills, both written and verbal * Experience working in a community setting * Experience working with individuals with complex needs * Ability and desire to work within an interdisciplinary team * Demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, and/or gender identity * Satisfactory Vulnerable Sector Check required * Valid driver?s license with $2 million vehicle liability insurance *Benefits including but not limited to*: * 4 weeks vacation time * Personal Leave * Education days and funding available * 12 hours of Appointment time per year * 15 hours of Inclement Weather time per year * 15 weeks of pregnancy leave top up * HOOPP (Healthcare of Ontario Pension Plan) * Medical and Dental Benefits * 7 hour per day working hours * 14 Statutory Holidays Please send a cover letter and resume to: *careers@ucfht.com by Tuesday, November 19, 2024 at 4:00pm* _We thank all applicants for their interest but only those being interviewed will be contacted. _ _Internal applications will be considered prior to external applications._ Please visit the Upper Canada Family Health Team website or the Upper Canada Family Health Team Facebook page for more information.
