Description PeaceHealth is seeking a Clinical Social Worker for a Part Time, 0.70 FTE, Day position. The salary range for this job opening at PeaceHealth is $44.25 - $66.37. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Job Summary The Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. Provides disposition and discharge planning, risk assessment, case management, and coordination of services to provide continuity of care. Details of- LocationVancouver, BC
