Vibrant continues the legacy of its parent organizations, Anne Johnston Health Station and Tobias Attendant Care, by providing primary and preventative healthcare and attendant services to the broader community. We are currently seeking a part-time (0.4FTE) *Community Dietitian*. This is a permanent position with HOOPP and benefits.
*Position Overview*
Reporting to the Manager, Health Services, the Community Dietitian serves a critical role within our communities and works within an interdisciplinary health promotion framework. The Community Dietitian provides education, skills building and support through individual nutritional counseling, group education, advocacy and outreach. As an inter-professional team member, the Community Dietitian fosters a culture of safety by promoting the well-being of clients and health care providers.

*Responsibilities*
· Conduct nutritional assessments and develop personalized treatment plans
· Design and lead community health programs focused on nutrition education and chronic disease prevention with an emphasis on community nutrition care and health promotion
· Build the capacity of individuals and populations to promote, maintain and restore health and prevent disease through nutrition and related means
· Work in collaboration with an interdisciplinary team to plan, organize, direct and evaluate clients? nutritional care
· Collaborate with public health and community-based organizations to provide access to health resources
· Lead health education initiatives to promote healthier lifestyles
· Adapt programs to meet the needs of diverse populations
· Assess, promote and enhance health and the prevention of nutrition-related disease using population health and health promotion approaches and strategies that focus on the intersection of the determinants of health, food security and overall health
· Conduct outreach activities
· Manage and comply with administrative duties as required by the organization as well as professional and legislative requirements
· Participate in other duties as assigned from time to time
*Qualifications *
· A university degree in food and nutrition or a related field from a recognized institution
· Completion of an accredited Dietetic Internship program
· Current registration with the College of Dietitians of Ontario (CSO), in good standing
· Membership with Dietitians of Canada
· Professional liability insurance is a requirement
· Knowledge of clinical guidelines and relevant community health resources
· Minimum 2 years in related field, both in community based and clinical settings
· Strong clinical and health assessment skills with a client-centred approach
· Ability to work effectively both independently and in interdisciplinary teams
· Experienced in use of digital tools (e.g. EMR, Zoom, TEAMS) for client care and program delivery
· Proficient in client education, counselling, and assisting with weight management, chronic disease
