*Job Summary*
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families facing various challenges. This role involves assessing client needs, developing intervention plans, and connecting clients with appropriate resources. The Social Worker will work collaboratively with other professionals to promote the well-being of clients and facilitate positive outcomes.
*Responsibilities*
* Conduct comprehensive assessments of clients' needs, strengths, and challenges.
* Develop individualized care plans in collaboration with clients and their families.
* Provide counseling and support to clients dealing with emotional, social, or financial difficulties.
* Advocate for clients' rights and access to necessary services.
* Maintain accurate and confidential client records in compliance with regulations.
* Collaborate with healthcare providers, community organizations, and other stakeholders to coordinate services.
* Monitor client progress and adjust care plans as needed.
* Participate in case management meetings and contribute to team discussions.
*Skills*
* Strong interpersonal skills with the ability to build rapport with diverse populations.
* Excellent communication skills, both verbal and written.
* Empathy and active listening skills to understand clients' needs effectively.
* Problem-solving abilities to develop effective intervention strategies.
* Knowledge of social services resources and community programs.
* Ability to work independently as well as part of a multidisciplinary team.
* Proficiency in case management software and documentation practices.
