We are currently looking to hire a:
*Community Health Team ? Community Pharmacist*
*1 ? Part-time, Permanent (14 hours/week)*
*Reporting To: *Manager, Interprofessional Primary Care
*Purpose: *
The pharmacist is a compassionate and caring interprofessional team member, supporting appropriate medication use for registered clients. The pharmacist works with the interdisciplinary team to complete medication reconciliations, review prescribed medications to identify medication-related concerns (interactions), provide drug information, consult with the multidisciplinary team as necessary, support medication reviews, create medical directives, develop and facilitate educational activities for staff and clients, and promote client safety through audits.
*Key areas of responsibility:*
Clinical
· Accurately input and update clients' medication information in electronic health records.
· Conduct medication reviews for complex cases and educate clients about proper medication use and associated side effects.
· Assist in creating and updating facility medication policies and procedures.
· Complete post-discharge medication reconciliation for clients.
· Review prescribed medications and recommend alternatives not covered by the Ontario Drug Benefit Program.
· Facilitate chronic disease education sessions and arrange follow-up appointments as necessary.
· Adjust client medications in consultation with the primary care provider or as per medical directives.
· Assist in deprescribing to reduce polypharmacy and the use of potentially inappropriate medications (PIMs), contributing to quality improvement and patient safety.
· Create and update medical directives for flu vaccinations and COVID-19.
· Order, monitor, and restock vaccines and medications as needed.
· Educate and serve as a resource for interdisciplinary teams and clients regarding medication use.
Health Education and Counseling
· Provide smoking cessation counseling.
· Coordinate stocking of vaccines, point-of-care testing (POCT), and vaccine refrigeration monitoring.
· Assist providers with new client intake by completing comprehensive medication reconciliations.
· Identify client and community needs, assisting in program and service development.
· Maintain accurate records in the electronic medical record (EMR) and participate in case reviews to support education.
· Participate in workshops to enhance skills and knowledge.
· Perform additional assigned duties within the role's scope.
Health Promotion
· Identify community needs and assist in program development.
· Engage in outreach and community work through planning, implementation, and evaluation.
· Participate in community development and partnership initiatives as needed.
Administrative
· Maintain complete and accurate electronic documentation.
· Participate in policy and procedure development.
· Engage in ongoing education workshops.
· Assist in resource selection and support student and volunteer placements.
· Contribute to the accreditation process and data collection initiatives.
· Design and deliver group programs as needed.
· Perform other reasonable duties as assigned.
