This is for a Long term Care. *Key Responsibilities:* * Assess, diagnose, and treat patients in a family medicine setting, including preventative care, health screenings, and ongoing management of chronic diseases. * Collaborate with the clinic?s multidisciplinary team to ensure comprehensive patient care. * Prescribe medications and order diagnostic tests as appropriate. * Educate patients on health management, preventive care, and wellness strategies. * Maintain accurate and up-to-date patient records in compliance with healthcare regulations. * Foster a positive, patient-centered environment to ensure high patient satisfaction and outcomes. *Qualifications:* * Registered Nurse Practitioner (NP) with the *College of Nurses of Ontario (CNO)*. * Strong clinical skills in assessment, diagnosis, and treatment. * Excellent communication and interpersonal skills. * Ability to work independently and as part of a collaborative healthcare team. * Compassionate, patient-focused approach to care.
