*Social Worker* The Chatham-Kent Family Health Team is seeking a dynamic experienced Registered Social Worker to join our team. The Social Worker will play an integral role in the care of patients of the Chatham Kent Family Health Team and will assist the FHT in achieving its vision and mission. The role of the Social Worker in the CK-FHT requires a thorough knowledge of mental health issues in a primary care context and the ability to function in a team environment with physicians, other interdisciplinary health professionals, and administrative staff. The role also requires a high level of interpersonal skills to deal with patients and their families in a holistic and compassionate fashion. Qualifications * Previous patient counselling in the primary health care setting * Exceptional oral, written, and presentation skills * Interpersonal skills * Group facilitation skills * In-depth knowledge of the local health program delivery and community based services * Able to be innovative, and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population * Ability to prioritize, manage time effectively and be flexible in a very active work environment Requirements *Master's Degree in Social Work or Counselling Psychology* * Registered currently with the Ontario College of Social Workers and Social Service Workers or College of Registered Psychotherapists of Ontario and able to attain Professional Liability insurance * Criminal background check, including vulnerable sector * _*Remuneration will be discussed with selected candidates*_ Please send cover letter and resume to Hiring Committee If a selected applicant requests any accommodation, we will consult with the applicant and provide, or arrange for the provision of, a suitable accommodation in a manner that takes into account the applicant?s accessibility needs due to disability.
